We invite staff from your Hospital and Health Service to attend the forum (CEO, Board Chair, Board Members, Executive, Managers and engagement staff) .
Who: Hospital and Health Service staff.
Price: We are offering eight (8) free registrations per HHS.

Support for Consumers to Attend
Health Consumers Queensland have some funds to support consumers to attend the forum who would otherwise be unable to participate due of the cost of travel. To help us distribute this fairly and openly, please complete this application form if you wish to receive travel assistance.
We have allocated a number of consumer registrations to each HHS and PHN. Speak to the engagement staff or key contact at the health service with which you are currently engaging with, about them supporting your attendance with travel assistance.
If you have made contact with your HHS or PHN and they are not able to cover your travel costs, please complete the Expression of Interest form, and Travel Application Form if you require travel support to attend.
Annual Forum Reference Group
We are seeking consumers/carers to join the Annual Forum Reference Group to help guide the organisation of the forum.
Role of 2017 Annual Forum Reference Group:
- To evaluate and make recommendations on submitted abstracts for 2017 Annual Forum
- To evaluate and make recommendations on consumer travel subsidy applications
- To provide input on the evaluation process

We invite presenters to submit abstracts for presentations focusing on the main theme; “Power and Passion: Culture change though consumer engagement and partnerships”.
