CheckUP, LTTS and Central, Wide Bay and Sunshine Coast PHN have co-designed and will be co-delivering a One Day (two session) workshop.  Each session are worth 4 hrs of CPD points.

 

Morning Session (8:30am - 12:30pm)

Patient Journey Story
      

The story of a patients journey (Content will discuss the patient journey through various stages of their lives).

 

Lunch (12:30pm - 1:30pm)

 

Afternoon Session (1:30pm - 5pm)       

Integrated Care Support

Integrated Care support to the patient journey (Content will discuss the funding sources available to allied health providers and who can do what to assist the client).
