    As prelude to the The Forum the networking dinner is an opportunity for CheckUP’s members, stakeholders and key primary health care professionals to enjoy a casual evening together, mingling with industry colleagues, meeting new faces and enjoying interesting discussion in the lead up to our signature event.

Refund & cancellation policy

Registrations for the CheckUP Forum Series close Friday 26 August 2016. Any cancellation of registrations will incur a $50 administrative fee. Cancellations received after Wednesday 17 August  2016 will not be entitled to a refund.  Registrations can be transferred to another appointed delegate at no extra charge.
